STEP 01

Confirm Eligibility

Age: 21 years or older. Minors require a parent or guardian to apply as a caregiver.
Residency: Must be an Arkansas resident with proof (e.g., AR driver’s license or state ID).
Medical Condition: Diagnosed with a qualifying condition such as:

• Severe Arthritis
• Fibromyalgia
• Alzheimer’s Disease
• Cachexia/Wasting Syndrome
• Peripheral Neuropathy
• Intractable pain (6 months+)
• Severe Nausea
• Seizures
• Severe Muscle Spasms

• Cancer
• Glaucoma
• HIV/AIDS
• Hepatitis C
• ALS
• Tourette’s Syndrome
• Crohn’s Disease
• Ulcerative Colitis
• PTSD

 Military Status: State law prohibits members of the Arkansas National Guard and United States Military from obtaining a registry ID card.

Obtain a Physician’s Written Certification

  •  Schedule an appointment with an Arkansas-licensed physician.
  • The physician must complete the official ADH Physician Written Certification form.
  • Ensure the certification is dated within 30 days of your application submission.

Gather Required Documents

Prepare the following:

  • A clear copy of your Arkansas driver’s license or state ID.
  •  The completed Physician Written Certification form.

Complete the Online Application

  • Visit the Arkansas Medical Marijuana Patient Registry System:
  • Register as a new user with a valid email address.
  • Fill out the application, upload required documents, and pay the $50 non-refundable fee using a debit or credit card (American Express not accepted).

Await Approval

  • Processing time is up to 14 business days from the date the ADH receives your complete application and payment.

STEP 05

Receive Your Medical Marijuana ID Card

  • If approved, you can print your card from the online portal.
  • A physical card will also be mailed to your address.

STEP 06

Your Medical Marijuana ID card is valid for one year from the date of issue. To maintain uninterrupted access, it’s recommended to begin the renewal process at least 30 days before expiration.
